Recently Dr. Oz has emphasize the need for L-Arginine supplementation for improved cardiovascular health and overall wellness. Here is a recent video of both Dr. Oz and Dr. Mercola talking about how important L-Arginine is in maintaining the flexibility and integrity of the vascular system. If you watched and listened to Dr. Oz and Dr. Mercola, they only talk about the essential amino acid L-Arginine. Technically they are correct from the standpoint of your endothelial cells using L-Arginine to create nitric oxide. Because nitric oxide relaxes the smooth muscle of the vascular wall to help maintain proper blood pressure, keeps blood platelet cells from sticking together to prevent heart attacks and stroke, and repairs the damage done to the endothelium to prevent atherosclerosis and arteriosclerosis, what Dr. Oz tells us about L-Arginine is critically important. But there are some other ingredients that need to be part of the equation, especially if you want to create nitric oxide in sufficient amounts to be therapeutic to your cardiovascular system. 

Dr. Oz, L-Arginine, and an Additional Key Ingredient! 

One of those ingredients needs to be the amino acid L-Citrulline. L-Citrulline recycles L-Arginine to have a synergistic effect on extending the endothelium’s ability to properly produce nitric oxide. Here’s what I mean. 

If you consume a therapeutic level of L-Arginine you will typically get a 30 minute to 2 hour window of improved nitric oxide production resulting in a small window of improved circulation. 

What Dr. Oz doesn’t tell you is critically important. If you combine your L-Arginine with a therapeutic level of L-Citrulline then you will increase this window of improved nitric oxide production to a 24 to 36 hour window. That’s huge especially in helping to repair your endothelium so that you can heal your vascular wall for long term improvements in your circulation. 

This means that you need both of these amino acids in sufficient quantities to truly obtain a benefit. To help you better understand this I’d like to provide two quotes from Dr. Louis J. Ignarro. Dr. Ignarro won the 1998 Nobel Prize in Medicine, along with two other American researchers, for discovering how your endothelium converts the essential amino acid L-Arginine into nitric oxide – the most powerful signaling molecule of your entire cardiovascular system. These quotes are taking from his 2005 bestseller NO More Heart Disease. 

My research shows that L-arginine in doses smaller than 4 to 6 grams produces almost zero increase in NO, so it is in essence an ‘all or nothing’ proposition – you must receive the full dose of L-arginine.” 

It is the synergy between the L-arginine (in a large enough dose), the L-citrulline, and the key antioxidants that creates dramatic increases in your body’s nitric oxide production. Without the proper combination of these nutrients, which so many other programs lack, you will receive little to no benefit from NO therapy.” 

Dr. Oz Doesn’t Tell You How Much L-Arginine to Use! 

While there is more I could say about additional ingredients, here are the two most important factors you need to consider in evaluating an L-Arginine product: 

  1. Does it include L-Citrulline? If it doesn’t, then you are wasting your time and money.
  2. Does it include sufficient amounts of both L-Arginine and L-Citrulline to create a synergistic and therapeutic benefit? According to Dr. Ignarro you need 4-6 grams of L-Arginine and 200 to 1000 mg of L-Citrulline to have a therapeutic effect. Anything less and you’re not going to get improved nitric oxide production. 

According to Dr. Ignarro, “Nitric oxide is the body’s natural cardiovascular wonder drug.” But for you to get a therapeutic effect to aid you in improving the health of your cardiovascular system, then you need to be taking both L-Arginine and L-Citrulline in sufficient quantities.  

There’s much more that could be said about the creation of nitric oxide; such as the need for Folic Acid and other antioxidants to enhance nitric oxide production over a long period leading to therapeutic benefits. In Part 1 of Strength Training we discovered how little attention is paid to the health of your endothelial cells and their importance to athletic performance. It doesn’t surprise me because most physicians and health care professionals don’t even talk about this subject with their patients.

Yet this organ called the endothelium and all its endothelial cells play a critical role in the health of your cardiovascular system. Every component of athletic performance centers on the health of your endothelial cells and their ability to properly produce nitric oxide, the master signaling molecule of the cardiovascular system.

In Part 2 we’re going to look at sports science and strength training and why you need to pay attention to your endothelial cells and their ability to produce nitric oxide.

Sports Science and Strength Training!

Sports science has come a long way in helping all levels of athletes improve their strength training. From teaching us the importance of developing your core muscles which provide a stable base for all your movements; to different strength training programs which improve muscle strength, tone, and endurance allowing you to stave off muscle fatigue and help to limit injury; to the importance of proper nutrition to both fuel and repair. Yet, there is still one factor that most leave out and that is the health of your endothelial cells and their ability to produce nitric oxide (NO).

According to Darryn Willoughby, PhD, CSCS, FISSN at Baylor University:

Arginine has been well researched and has many beneficial effects, especially in terms of cardiovascular health. Its main mechanism of action lies in boosting NO. NO is a signaling molecule within muscle cells that may have many anabolic effects, including increased nutrient transport and vasodilation. Arginine boosts nitric oxide by stimulating nitric oxide synthase, the enzyme that makes NO. Research suggests it may help improve exercise performance, support protein synthesis, boost growth hormone levels at higher doses, and even help replenish postworkout glycogen stores.

What Dr. Willoughby is pointing out is that nitric oxide plays a significant role in all phases of strength training. And, the organ of the body that is key to producing it is your endothelial cells that make up your endothelium. It is foundational to the cardiovascular system’s ability to deliver the necessary oxygen and nutrients needed for muscular contraction and relaxation. Additionally, that same cardiovascular system is needed to remove the waste products created during energy production so that the muscle and organ systems can work optimally or as close to optimally as possible.

Many serious strength training athletes spend a lot of time, money, and effort on training, nutrition, and equipment. Equally important is spending time making sure your endothelial cells are properly repaired and supplied with the amino acids they need for improved nitric oxide production.

Why Pay Attention to the Endothelial Cells and Nitric Oxide Production for Strength Training?

As promised I’m going to open your eyes and mind to information that can take you to a whole new level of strength training. But first a little background.

Your endothelium is a thin layer of cells that line the interior surface of your entire cardiovascular system including your heart. These cells are called the endothelial cells. When added up, the volume of these endothelial cells would cover the surface area of 8 tennis courts and weigh as much as the liver. That’s amazing since the endothelium is only one cell thick and can’t be seen by the human eye.

Once thought to be just an inert membrane, research over the last 25 years has shown that the endothelium is an active, multi-functional organ that plays a vital role in metabolic, immunologic, and cardiovascular health. Most researchers today would tell you that your endothelial cells ultimately determine the health of your blood vessels. It’s these blood vessels that are delivering the needed oxygen and nutrients for your athletic performance as well as removing the waste products that help determine the speed of your recovery. All key to strength training.

One critical function of the endothelial cells is facilitating the formation of new capillaries. While this is important in wound healing, it also plays a significant role in muscle creation and in the heart’s ability to develop collateral vessels to improve delivery of oxygen and nutrients. This collateral vessel formation is also key to improving your ability to fuel your muscles with oxygen and nutrients as well as effectively removing waste products.

Another critical function of the endothelial cells is the production of nitric oxide. Nitric oxide is the master signaling molecule of the cardiovascular system. It regulates the muscle tone of your blood vessels allowing them to dilate for improved blood flow into the working muscles, tissues, and organs. Nitric oxide stops blood platelet cells from grouping together to enhance blood flow. Nitric oxide transmits messages between nerve cells. Nitric oxide is associated with the process of learning and memory for improved cognitive function so that your head is clear and focused in the gym. According to Dr. Jonathan S. Stamler – Professor of Medicine at Duke University Medical Center:

It does everything, everywhere. You cannot name a major cellular response or physiological effect in which [nitric oxide] is not implicated today. It’s involved in complex behavioral changes in the brain, airway relaxation, beating of the heart, dilation of blood vessels, regulation of intestinal movements, function of blood cells, the immune system, even how fingers and arms move.”

From airway relaxation for improved lung function to dilation of the blood vessels for improved delivery of oxygen and nutrients to the muscles, tissues, and organs, nitric oxide plays a critical role in strength training.

In Part 3 we are going to examine how your endothelial cells and their ability to properly produce nitric oxide impacts your training, performance, and recovery in strength training. This final part will help you understand how you can experience substantial and significant improvements in your overall strength and fitness; but only if you nourish your endothelial cells properly.

Nitric Oxide and Diabetes!

DiabetesDiabetes has become a national health problem.  Fortunately, there are clinical studies to show that nitric oxide can positively affect the complications of diabetes.  This blog will help you understand the hard facts, complications, and the potential of a proper nitric oxide / l-arginine protocol to help with this devastating disease. 

Now before we start you might be wondering why an endocrine disease is showing up on a web site focused on cardiovascular health.  The answer is that diabetes directly affects the vascular system giving rise to most of the complications.  One noted clinical expert in this area, Dr. J. Joseph Prendergast says,

“Many people still think diabetes is a disease about sugar.  It’s not the sugar!  It’s the complications!”

Dr. Prendergast is not discounting the need to control your blood sugar.  That’s extremely important.  What he’s trying to get people to understand is that unregulated glucose (blood sugar) causes high levels of oxidative stress which damages the endothelial cells and their ability to properly produce nitric oxide – the master signaling molecule for your cardiovascular system.  To prevent the complications you also need to repair the damage to the endothelium.         

The Hard Facts!

Diabetes is a disease in which the body does not produce insulin (Type 1) or does not properly respond to insulin (Type 2).  According to the most recent statistics from the Diabetes Association of Greater Cleveland (updated in February of 2009)17.9 million American have been diagnosed with diabetes.

What makes this “America’s largest healthcare epidemic” is that 57 million Americans are in a pre-diabetic condition where their blood glucose levels are higher than normal but not high enough to be diagnosed with type 2 diabetes. 

Almost 25% of the American population is either in a pre-diabetic or diabetic condition and this is creating an enormous health challenge. 

Type 2 diabetes in adolescents has increased 10 times over the last decade and now represents 33% of new pediatric diabetes cases where as 20 years ago it was only 2%.  This diabetic problem is even worse for African Americans since they are 1.8 times as likely to develop diabetes as whites.  And, the death rates for African Americans with diabetes are 27 percent higher than for whites.  It is estimated that the annual health care costs of a type 2 diabetic is 2.3 times greater than a person without this disease.

Complications of Diabetes!

If you have diabetes or if you have a family member or friend who has diabetes than you already know how challenging this disease can be.  According to the Diabetes Association of Greater Cleveland:

  • Diabetes is a leading cause of adult blindness, lower-limb amputation, kidney disease and nerve damage.
  • 40% of diabetics suffer some degree of hearing impairment.
  • 66% of diabetics die from a heart attack or stroke.
  • 28% of diabetics develop kidney disease
  • 23% of diabetics have foot problems including numbness and amputations.

To put these numbers in perspective, every 24 hours 4,100 new cases of diabetes are diagnosed, 810 people will die, 230 will have a diabetes-related amputation, 120 new patients will require kidney dialysis or transplant, 55 will go blind, and nearly 65% of diabetics will die from cardiovascular disease.

How Nitric Oxide Helps!

Although there is no evidence to date that shows that Nitric Oxide (NO) improves insulin availability, there is plenty of evidence that NO can intervene to help prevent diabetic complications.  A family of enzymes called Nitric Oxide Synthase (NOS) generates NO from the essential amino acid L-arginine.  NO is the master signaling molecule for the cardiovascular system, is an important neurotransmitter, helps to mediate pain, and is necessary in wound healing and tissue repair.

Unfortunately, NO production is often impaired in both Type I and Type II diabetics.  Current research seems to indicate three limiting factors:

Limiting Factor 1 is an accumulation of asymmetrical dimethly arginine or ADMA in the blood.  Under normal metabolism of L-arginine a small amount of a natural inhibitor to the NOS enzymes is formed called ADMA.  Normally, ADMA doesn’t accumulate because it is eliminated in the urine through normal kidney functions.  However, reduced kidney function is part of aging and both Type I and Type II diabetes accelerates this kidney dysfunction.  This allows ADMA to accumulate in the blood stream and inhibit the NOS enzymes to reduce NO production.

Limiting Factor 2 is a change in pH from alkaline to acidic.  The NOS enzymes are pH dependent and work best in slightly alkaline (basic) conditions.  In diabetes, glycolysis and ketoacidosis are negative factors that force tissue pH towards acidic conditions.  To compensate the body will use calcium to restore its proper alkaline pH.  Calcium is needed to activate the NOS enzymes.  Thus, this acidic change causes the NOS enzymes to become less active and efficient resulting in a decrease in NO production.

Limiting Factor 3 is adequate supplies of oxygen necessary for the NOS enzymes and NO production.  Since diabetics typically have impaired circulation this reduces blood flow and the body’s ability to supply oxygen to the endothelial cells particularly in the extremities.  Additionally, diabetics often experience elevated blood glucose levels.  This extra glucose becomes attached to the hemoglobin to change its structure and to bind NO instead of oxygen.  Once NO is bound to the hemoglobin it is not easily released which compounds the problem.

Diabetes creates a compounding effect which leads to a multitude of complications.  As this disease causes biochemical changes in the blood stream, kidneys and surrounding tissues thru low oxygen, acidosis, and the accumulation of ADMA it reduces the production of NO.  The reduction in NO reduces wound healing and tissue repair (especially affecting the endothelial cells).  This reduction also limits normal vasodilation which affects the cardiovascular system.  This continues to impair the production of NO which negatively affects neurotransmission and pain.  Thus, many of the complications of diabetes such as heart disease and high blood pressure, retinopathy (eye problems), neuropathy (nerve damage), kidney disease, bruises to the hands, feet and legs, and poor circulation to the extremities (which often lead to amputation) are all aggravated by low NO production.

Two Fold Approach to Improved Diabetic Health!

Most people who deal with diabetes only deal with blood sugar levels.  There is no question that this approach is absolutely necessary.  Unfortunately, the oxidative damage done to the endothelial cells is not addressed.  Yet addressing this problem is critical to reducing the risk for diabetic complications.  Again, I refer back to Dr. J. Joseph Prendergast and his comment at the beginning of this post:

“Many people still think diabetes is a disease about sugar.  It’s not the sugar!  It’s the complications!”    

Dr. Prendergast has 35+ years of experience as a prominent Endocrinologist in Palo Alto California with an extremely large patient base of diabetics.  His clinical studies have shown that if you implement a proper Nitric Oxide / L-arginine protocol you can effectively reduce and/or eliminate much of the oxidative stress to the endothelial cells.  This results in good NO production to eliminate or reduce many diabetic complications.  For example, of the 7,000 patients who have followed his Nitric Oxide / L-arginine protocol none have had serious hospitalizations, no amputations, and less than 1% has had cardiovascular issues.

Monitoring blood sugar levels will always be important for proper diabetic care.  Equally important is properly nourishing your endothelial cells through diet (please read my article about the Acid – Alkaline Balance), exercise, and taking sufficient amounts of L-arginine.  Each of these three lifestyle factors will go a long way to helping your endothelial cells stay healthy and produce the necessary Nitric Oxide to prevent the complications of diabetes.
